Health Requirements Overview

Parents must submit the physical examination and proof of immunization forms by the first day of school; students who do not have those health forms submitted will not be able to attend school.

Vision and Hearing Screenings

Vision and Hearing Screenings are mandated by the State of Illinois and provided in accordance with the Illinois Department of Health guidelines. 

Annual screenings are done on the following schedule.

Hearing:

All students receiving special education services, students new to the district, students in Pre-K, Kindergarten, 1st, 2nd, and 3rd grade, as well as students referred for screening by teachers or parents. 

Vision:

All students receiving special education services, students new to the district, students in Pre-K, Kindergarten, 2nd, and 8th grade as well as students referred for screening by teachers or parents. 

Parents/ guardians and staff members who suspect a problem with a student's vision or hearing are encouraged to contact the school health office to request a screening. Students who do not pass the school screenings are referred for medical follow up. 

Parents or guardians who object to the screenings for their child on religious grounds may contact the school nurse for additional information.
